Requesting Immunization Records
The Georgia Registry of Immunization Transactions and Services (GRITS) contains immunization records submitted to the Department of Public Health Services by healthcare providers across the state. If an individual received vaccinations—such as flu, tetanus, or others—after inception of the registry in 2003, there is a strong possibility the immunization information is included in the registry. You may also contact your last physician or healthcare facility where you received immunizations for assistance.
To request a copy of an immunization record, individuals must submit an online request to Georgia Department of Public Health using the Online Immunization Record Request link.
Once GRITS receives the completed request, a search is typically conducted within three to five business days. The registry will then respond with the results of the search.
